HealthRise is a five-year, $17 million global program funded through Medtronic Philanthropy. Its aim is to contribute to a reduction in premature mortality associated with cardiovascular disease (CVD) and diabetes by 25% among underserved populations in its areas of operation. Its primary objectives are to (1) increase screening and diagnosis of CVD and diabetes; and (2) increase management and control of these conditions. HealthRise works in South Africa, India, Brazil and the United States. In each country, it supports innovative demonstration projects that empower people living with these diseases, builds the capacity of frontline health workers to address the needs of these populations more effectively, and supports advocacy and policy. Abt Associates coordinates HealthRise global and country-level programs, and the Human Sciences Research Council (HSRC) manages the South Africa program. The Institute for Health Metrics & Evaluation (IHME) leads HealthRise needs assessment and monitoring and evaluation with support from Social Survey Africa. In South Africa, the HSRC is regarded as the implementation partner providing technical oversight to organizations that will conduct the implementation of the selected interventions. The roles of the implementation partner and the evaluation partner are clearly defined and as such, the implementation, and monitoring and evaluation activities will be conducted independently of each other.
